Pursuant to Articles 54 and 58 and Article 57(4) of the Lugano Convention, the court or other competent authority shall, upon request, provide a certificate confirming the enforceability and, where applicable, the date of service of the document instituting the proceedings, using the forms set out in Annexes V and VI to the Lugano Convention.
